MATCH Program

MATCH, or Medical Assistance Through Community Healthcare, is a pilot program that grew out of research done by the LBJ School of Public Affairs at the University of Texas at Austin.

The LBJ School’s research revealed that 13 percent of uninsured patients who are released from the hospital are readmitted within 90 days. The uninsured often do not have a primary care provider and are unable to get the necessary medications and follow-up treatments after a hospital stay, which leads to them returning to the hospital.   

MATCH is trying to help these uninsured patients transition to community clinics after they leave the hospital. During the transition period, patients can receive medical treatment from MATCH staff.

The MATCH program’s goal is to help these uninsured patients find a medical home where they will have access to medications, health education and other medical services to help them achieve optimal health.

The program is currently in a pilot stage and is only able to treat patients from St. David’s Medical Center. We are unable to take other requests for treatment or placement at this time.

 

MATCH Mission Statement:
To reduce the risk of admissions, readmissions and emergency room encounters for uninsured patients in Central Texas, and to improve their long-term health and well-being by matching them with primary and preventative care resources.
